What is a Community Health Worker (CHW)?

According to the American Public Health Association, A Community Health Worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.”

Great Rivers HUB is one of just three validated Core Competency training sites in Wisconsin. All CHWs connected to Great Rivers HUB receive C3 Core Competency training, Pathways Community HUB model training, and training for documenting in the HUB’s database, Care Coordination Systems (CCS). Our CHWs are also encouraged to take advantage of local Resilient & Trauma-Informed Community trainings. CHW backgrounds range widely, each leveraging their own experiences to provide trauma-informed and holistic care to the individuals they serve.

  • Are you interested in becoming a CHW? Great Rivers HUB is offering two Core Competency Training (pdf) cohorts in 2025:
    • Monday, March 10, through Friday, April 11
    • Monday, September 8, through Friday, October 10
